Request for Action:  Minerva-Request For Action Form

The above form should be used by the Circulation and Cataloging Standing Committees. Please fill it out and submit it to the Minerva  Executive Board for action at the Board’s next meeting.

Intra-Minerva Pattern Tracking

This form should be used by Minerva Libraries to report issues between other Minerva libraries. The data will be used to examine trends and advise the Executive Board if training, policy adjustment, or other formal action that needs to be taken. All issues, whether resolved or not, should be listed to aid in the best data collection. Submissions are confidential and will only be used by designated people to do follow up as needed.
